I found this in Dads' old tackle box. No one in the family remembers when or where it was used. Any ideas?

Don't know the lure, but that is the tiger pattern. I had a floating mouse and a jointed minnow in that pattern.
With that bill, it should be a deep diver.

After a few casts I see that it is a floater/diver. It gets down to around 6 feet on a speedy retrieve.

I Googled the Heddon Tad polly. Interesting lure.

There is a logo or trademark molded into the lure but I can't make it out.
